Delayed rupture of a PSA is a rare but life-threatening complication of high-grade splenic injury. In polytrauma patients, the diagnosis may be difficult as pain may mask signs of intra-abdominal hemorrhage. This case underscores the need for continued vigilance during rehabilitation to identify and manage delayed complications.
